  • Looking for CPT and HCPCS Code Tables or a related covered diagnosis? Per CMS CR-10901, codes were relocated from the LCDs into their corresponding articles. If you don’t see the code inside and LCD, be sure to check its associated article, linked at the bottom of the LCD document, which will open in a new tab.

    LCD # - This is the best way to search. If you know the LCD #, for example, "L35006", simply enter that the number.

    CPT/HCPCS Code Search - If you don't know the LCD #, try a procedure code, such as: "78815". Do not enter extra keywords such as "CPT 78815" or "78815 Pet scan". This search option is not available for NCDs since the procedure and diagnosis codes are listed on a separate attachment.

    ICD-10 Diagnosis Code Search - If you don't know the LCD #, try a diagnosis code, such as "E08.00" only. Do not enter extra keywords such as "ICD -10 E08.00" or "E08.00 Diabetes mellitus". This search option is not available for NCDs since the procedure and diagnosis codes are listed on a separate attachment.
    Note - ICD-10 codes that are included inside a range of codes will not pull up in the search.

    Keyword - When you don't have an LCD #, or procedure code, try using a descriptive term like "therapeutic exercise" rather than "physical therapy"

    Multiple Keywords - Are you looking for a multiple-word search phrase, like "hyperbaric oxygen therapy?" Change the drop-down menu, next to the search box, to Exact Phrase Match. Make sure to check your spelling.

    Preventive Service codes - Please view the for coverage guidelines. The chart includes Healthcare Common Procedure Coding System (HCPCS)/Current Procedural Terminology (CPT) codes; diagnosis codes; coverage requirements; frequency requirements; and beneficiary liability.

    Older Dates of Service - If you are looking for older dates of service and if the results page indicates no records returned for Articles and/or Local Coverage Determinations (LCDs) try using the MCD Archive Site link located under the MCD Archive heading. Use Search or the other available options on the left of the screen.
