Submit an appeal to original Medicare:
Use the
CMS – 20027 form (version 01/20) and send a copy of the completed form to the company that handles your claims for Medicare. Their address is listed in the "Appeals Information" section of the MSN;
or Send a copy of the completed
Medicare summary notice to the MAC listed on your MSN. Their address is listed in the "Appeal Information" section of the MSN.
