This checklist is intended to provide healthcare providers with a reference for use when responding to additional documentation requests for E/M services (on or after 1/1/2021 for office/outpatient services and on or after 1/1/2023 for all other services). Healthcare providers retain responsibility to submit complete and accurate documentation.
